چکیده

Implementation of Image-guided adaptive brachytherapy (IGABT) in India and other developing countries is a major challenge due to the resources, expertise and financial constraints. The basis of this thesis was to address various issues related to transition and successful implementation of IGABT in clinical routine.[1] Tata Memorial Hospital (TMH), Mumbai has a long tradition of being active in international collaborative research. Various fellowship programs for both clinicians and physicists have been the basis for successful collaborative research. Within many such projects, TMH is one of largest participating centers in the ongoing multi-centric collaborative international study on magnetic resonance (MR) imaging-guided brachytherapy in locally advanced cervical cancer. The study was designed to evaluate the dose-effect relationship on local control/morbidity of the tumor and toxicities of organs at risks (OAR) when MR images were used for cervical cancer brachytherapy. This thesis reiterates that, like any other advanced techniques, IGABT too requires systematic clinical implementation taking into account various technological, dosimetrical, and clinical issues. Systematic transition and appropriate implementation of IGABT for locally advanced cervical cancers may result in better outcome.

Design and Implementation of a Complementary Treatment Planning Software for the GZP6 HDR Brachytherapy System (GZP6 CTPS)

Introduction: Brachytherapy is one of the most common treatment modalities for gynecological cancer. The GZP6 brachytherapy system is one of the devices utilized in Iran. It has been considered particularly due to its low cost compared to other more complete and established systems. This system has some deficiencies including lack of a treatment planning software for non-predefined treatments, ...

Can combined intracavitary/interstitial approach be an alternative to interstitial brachytherapy with the Martinez Universal Perineal Interstitial Template (MUPIT) in computed tomography-guided adaptive brachytherapy for bulky and/or irregularly shaped gynecological tumors?

BACKGROUND Interstitial brachytherapy (ISBT) is an optional treatment for locally advanced gynecological tumours for which conventional intracavitary brachytherapy (ICBT) would result in suboptimal dose coverage. However, ISBT with Martinez Universal Perineal Interstitial Template (MUPIT), in which ~10-20 needles are usually applied, is more time-consuming and labor-intensive than ICBT alone, m...

Developing a Verification and Training Phantom for Gynecological Brachytherapy System

Introduction Dosimetric accuracy is a major issue in the quality assurance (QA) program for treatment planning systems (TPS). An important contribution to this process has been a proper dosimetry method to guarantee the accuracy of delivered dose to the tumor. In brachytherapy (BT) of gynecological (Gyn) cancer it is usual to insert a combination of tandem and ovoid applicators with a complicat...

Three-dimensional transrectal ultrasound guided high-dose-rate prostate brachytherapy: A comparison of needle segmentation accuracy with two-dimensional image guidance.

PURPOSE Conventional transrectal ultrasound guided high-dose-rate prostate brachytherapy (HDR-BT) uses an axially acquired image set for organ segmentation and 2D sagittal images for needle segmentation. Sagittally reconstructed 3D (SR3D) transrectal ultrasound enables both organ and needle segmentation and has the potential to reduce organ-needle alignment uncertainty. This study compares the ...

Evaluation of deformable image registration in HDR gynecological brachytherapy

Introduction: In brachytherapy, as in external radiotherapy, image-guidance plays an important role. For GYN treatments it is standard to acquire at least CT images and preferably MR images prior to each treatment and to calculate the dose of the day on each set of images. Then, the dose to the target and to the organs at risk (OAR) is calculated with worst case scenario from I...
